f53df7da64 clang-format: Apply formatting
Code submissions have continually suffered from formatting
inconsistencies that constantly have to be addressed.  Using
clang-format simplifies this by making code formatting more consistent,
and allows automation of the code formatting so that maintainers can
focus more on the code itself instead of code formatting.

286e43bba8 win-wasapi: Speaker enum fixes
Remove KSAUDIO_SPEAKER_2POINT1 because it is included in the Windows
SDK. (I'm guessing it wasn't before.)

Redefine 4POINT1 as KSAUDIO_SPEAKER_SURROUND|SPEAKER_LOW_FREQUENCY
instead of KSAUDIO_SPEAKER_QUAD|SPEAKER_LOW_FREQUENCY. See definitions:

FFmpeg 4.1: FL+FR+FC+LFE+BC
WASAPI QUAD: FL+FR+BL+BR (looks wrong)
WASAPI SURROUND: FL+FR+FC+BC
WASAPI LOW_FREQUENCY: LFE

69b4df41c6 win-wasapi: Unapply/reapply audio monitoring on reconnect
Saves and disable monitoring before entering recover loop, and restores
it after recovery is complete.

041d3b4155 win-wasapi: Fix audio capture after unplugging device
This commit fixes a bug where the user would have to restart OBS when
certain types of devices are unplugged and plugged back in again
(particularly USB devices).  A wait interval is required
(RECONNECT_INTERVAL) because Windows does not send any events to
IAudioCaptureClient on device invalidation.

67e48ecc2c libobs/media-io: Replace quad with 4.0
(also obs, deps/media-playback, libobs/audio-monitoring, decklink,
linux-alsa, linux-pulseaudio, mac-capture, obs-ffmpeg, win-dshow,
win-wasapi)

Default channel layout for 4 channels is 4.0 in FFmpeg.
Replacing quad with 4.0 will improve compatibility since FFmpeg has
better support of its default channel layouts.

fb580535c9 libobs/media-io: Clean surround API
(also modifies obs-ffmpeg, audio-monitoring, win-wasapi, decklink,
obs-outputs)

Removes speaker layouts which are not exposed in UI.  The speaker
layouts selectable by users in the UI are the most common ones.  It is
not necessary to keep other layouts.  (This basically removes
5POINT1_SURROUND, 7POINT1_SURROUND, SURROUND =3.0).

bbac3280c1 libobs: Add surround sound audio support
(This commit also modifies the following modules: UI,
deps/media-playback, coreaudio-encoder, decklink, linux-alsa,
linux-pulseaudio, mac-capture, obs-ffmpeg, obs-filters, obs-libfdk,
obs-outputs, win-dshow, and win-wasapi)

Adds surround sound audio support to the core, core plugins, and user
interface.

Compatible streaming services: Twitch, FB 360 live
Compatible protocols: rtmp / mpeg-ts tcp udp
Compatible file formats: mkv mp4 ts  (others untested)
Compatible codecs: ffmpeg aac, fdk_aac, CoreAudio aac,
		   opus, vorbis, pcm (others untested).
Tested streaming servers: wowza, nginx
	 HLS, mpeg-dash : surround passthrough
Html5 players tested with live surround:
	 videojs, mediaelement, viblast (hls+dash), hls.js
Decklink: on win32, swap channels order for 5.1 7.1
         (due to different channel mapping on wav, mpeg, ffmpeg)
Audio filters: surround working.
Monitoring: surround working (win macOs linux (pulse-audio)).
VST:	 stereo plugins keep in general only the first two channels.
	 surround plugins should work (e.g. mcfx does).
OS: win, macOs, linux (alsa, pulse-audio).
Misc: larger audio bitrates unlocked to accommodate more channels
NB: mf-aac only supports mono and stereo + 5.1 on win 10
         (not implemented due to lack of usefulness)

fc61e6ed69 win-wasapi: Fix timestamp calculation
When device timing is used, it shouldn't be modifying the timestamp.
Fixes an issue where certain devices with large audio segments would
seem a bit out of sync.

5b20942247 win-wasapi: Subtract frame duration from timestamp
It would appear that the timestamp values returned by devices are not
perfectly accurate, and in some cases may be calculated on the spot --
to combat that, it's best to subtract the audio segment's duration from
the current audio segment's timestamp to ensure the timing is as
accurate as possible.

e006d961a4 audio-monitoring: Add ability to monitor Outputs
(Note: This commits also modifies the linux-pulseaudio, mac-capture, and
win-wasapi plugins)

Do not prevent the targeted output device from being monitored if the
selected monitor output device is a different one.

39d76cc76f win-wasapi: Mark audio outputs as unmonitorable
(Note: This commit also modifies mac-capture and linux-pulseaudio)

This prevents outputs from being monitored, preventing a potential
feedback loop.

cd97ce2a17 libobs: Add source output flag OBS_SOURCE_DO_NOT_DUPLICATE
Certain types of sources (display captures, game captures, audio
device captures, video device captures) should not be duplicated.  This
capability flag hints that the source prefers references over full
duplication.

18542461f2 win-wasapi: Use system timestamps by default for input
Microphones and other input devices can often have bad or erroneous
timestamps.  Although we handle bad timestamps much better in
obs-studio, there are still lingering issues that can crop up from time
to time with device QPC timestamps that leads to mic data not playing
back properly.  It's best if it be off by default rather than on, which
will now cause it to use system timestamps for input devices by default.
This changes it to the same handling as OBS1 for this case.

obs_output_info::get_name(void)
obs_encoder_info::get_name(void)
obs_service_info::get_name(void)

API changed to:
obs_source_info::get_name(void *type_data)
obs_output_info::get_name(void *type_data)
obs_encoder_info::get_name(void *type_data)
obs_service_info::get_name(void *type_data)

This allows the type data to be used when getting the name of the
object (useful for plugin wrappers primarily).

NOTE: Though a parameter was added, this is backward-compatible with
older plugins due to calling convention.  The new parameter will simply
be ignored by older plugins, and the stack (if used) will be cleaned up
by the caller.
