Change from dimension2d<s32> to dimension2d<u32> where practical. IImage and ITexture now have unsigned dimensions, as does screen size. This has had a significant knock-on through the code base, but it is more technically correct - the BEST KIND of correct.
GUI elements and draw2DImage methods still have and take signed dimensions, but we can look into this in future. Regression tested with the test suite and the example apps. Note that createDevice() now takes dimension2d<u32>!

Make joystick activation optional, so that Irrlicht doesn't consume all joystick events. This allows us to pass joystick enumeration data to the user app. Revert example 04 to just do key movement, but have it show framerate independent movement. Add a new 19.MouseAndJoystick example. Tested with Win32 and Linux devices, and with the SDL device on Windows and Linux.

Changed the device creation to make more use of SIrrlichtCreationParameters. This should increase maintainability and ease addition of more configurable properties. The windows devices need to be adapted still, but should compile for now...

Some more constification, but still not done. Now we have also SEvent in OnEvent as const ref, so most user apps will need some kind of upgrade for the new library version.
