Fix Bloomberg/Haaretz.com/Nikkei Asian Review (banners) + readme update

Remove sticky subscription/page counter banners on Bloomberg, Asia Nikkei & Haaretz.com.
Add 'don't remove the extension's folder after installation' in readme.md
