UI: Don't recreate entire Hotkey Settings tab

This replaces the programmatic generation of the hotkeys tab with XML.
Also fixes a memory leak. Reuses concepts from LoadAudioSources().

This changes cleans up some deprecated functions that were removed in
Qt6. Some are placed behind version ifdefs and others are replaced for
their non-deprecated Qt5 version.

UI: Replace deprecated QLayout->setMargin with setContentsMargin
...
QLayout->setMargin has been deprecated since Qt 5.13 (though marked
obsolete since at least Qt 4.8) and removed in Qt6. It was replaced by
QLayout->setContentsMargins, which is available in all versions of Qt5.
Building against Qt 5.13+ can produce compiler warnings when using
QLayout->setMargin, and warnings are bad.

clang-format: Apply formatting

Code submissions have continually suffered from formatting
inconsistencies that constantly have to be addressed. Using
clang-format simplifies this by making code formatting more consistent,
and allows automation of the code formatting so that maintainers can
focus more on the code itself instead of code formatting.

disable IME for OBSHotkeyEdit.
