Rewriting headers and updating all content to match.

This implementation should be much saner and less buggy.
Changes:
- Using header rows is now mandatory
- Headers now stretch to fit their content, rather than needing to
specify a header height
- Tabs should now be inside the header

For un-upgraded spinners, the behavior without `is-active` is
inconsistent with the upgraded behavior. With this patch it's

Makes typography definitions optional, through mdl-typography class.

Also adds mdl-base class, which is currently set to include just the
mdl-typography definitions, but will later be extended to include
other global definitions.
All demos, tests and templates have been updated to use mdl-base.

Adding classes for setting different cell sizes at different screen sizes.

So class="mdl-cell-2-col mdl-cell-4-col-phone" will get 2 columns at
desktop and tablet sizes, and 4 at phone size.
I'm slightly worried by the fact that we're enabling this while also
changing the total number of columns at different screen sizes. Will
developers grok this, or should we just make it 12 columns across all
screen sizes?
